Electronic Transcript Request
ÍøÆØ³Ô¹Ï has authorized the National Student Clearinghouse to send electronic transcripts.
- Electronic transcripts cost $2.50-$3.50 each, payable by debit or credit card. Transcripts are free for currently enrolled students sending transcripts to other .
- Once your order is placed, you will receive an order confirmation number via email. To , enter your order confirmation number and email address used to place the order. Please contact the if you need help or have questions about your order.
- If you attended ÍøÆØ³Ô¹Ï prior to Fall 1986, you may not have an electronic transcript available and will need to fill out the Records Request Form below.
Paper Transcript and Other Records Request
If you are requesting a physical transcript to be mailed or faxed, or would like to pick up your records in the Admissions and Records Office, please fill out the ÍøÆØ³Ô¹Ï Student Records Request form.
You can request the following documents via this form:
- Official ÍøÆØ³Ô¹Ï transcript
- Unofficial ÍøÆØ³Ô¹Ï transcript
- ÍøÆØ³Ô¹Ï class schedule
- ÍøÆØ³Ô¹Ï acceptance letter
- Other unofficial records in your student file (transcripts from other schools, immunization records, etc.)
Enrollment Verification
Enrollment Verification letters generally consist of a typed letter that includes the semester (including dates) and full-time or part-time status. If any additional information needs to be included in the letter, please include it in the top section of the form.
If you need additional forms filled out, please complete our Enrollment Verification Form and attach the necessary form(s). We cannot fill out blank forms, so be sure to fill out all sections other than those that need to be completed by the Admissions and Records Office.
We are only able to verify enrollment for current or past terms, not future terms.
Change of Major
If you are changing your major from non-degree seeking to degree seeking, your major will not be changed until we have received official transcripts from all institutions.
Change of Student Information
Change of Student Information Form
The Change of Student Information form can be used to change any of the following information:
- Address (required document: state-issued ID or Driver’s License)
- Name (required documents: two forms of identification including Driver’s License, birth certificate, marriage license, or Social Security Card with your updated name)
- Phone number
- Email address
- Social Security Number (required document: signed Social Security Card)
Visiting Students
Visiting Student Approval Form
Students who are currently enrolled at another institution and want to take courses at ÍøÆØ³Ô¹Ï to transfer back to their current college or university are considered “transient students.”
Transient students are required to complete the Visiting Student Approval Form with a college official from their current institution before enrolling in course(s) at ÍøÆØ³Ô¹Ï.
In-State Residency
If you are flagged as out-of-state and wish to receive in-state tuition, you can submit an In-state Residency Application and supporting documentation.
To receive in-state residency status, this application and supporting documentation must be submitted before the last day to register for classes for that term. Incomplete applications or applications received without documentation WILL NOT be reviewed, and out-of-state fees will remain on your account until Tennessee residency has been determined.
Academic Fresh Start
Academic Fresh Start Application
Academic Fresh Start forfeits college credits with an “F” grade from GPA calculation. If your degree program requires a “C” grade or higher, “D” grades will also be forfeited. The courses and grades will remain on your transcript but will be removed from the GPA calculation and degree requirements. All credit hours will be included in attempted credit hour calculations.
Only students that were enrolled from Spring 2020 - Summer 2021 are eligible for COVID Fresh Start. Students enrolled in Fall 2021 are not eligible to apply.
